Male factor infertility accounts for roughly half of all fertility challenges, yet most fertility workups focus almost entirely on the female partner until a semen analysis comes back abnormal.By then, months have often been lost.

​

Sperm is not a fixed parameter. Unlike eggs, which are present from birth, sperm are produced continuously on a 74-day cycle.

This is the single most important fact in male fertility treatment: if you start now, the sperm present in three months will be measurably different from the sperm present today.

 

Acupuncture and Chinese herbal medicine work directly within that 74-day window to improve the conditions in which sperm develop.

What Acupuncture Treats in Male Fertility
 

Low sperm count (oligospermia)

Sperm count below 15 million per milliliter is classified as oligospermia. Causes include varicocele, hormonal imbalance, heat exposure, oxidative stress, and lifestyle factors.

Chinese medicine addresses low count primarily through kidney yang and jing tonification: the framework that maps most closely onto spermatogenesis and testicular function.

Herbal formulas used for this pattern have documented effects on sperm concentration in clinical trials.

Poor sperm motility (asthenospermia)

Motility is the percentage of sperm moving and the quality of that movement. It is often more predictive of fertility outcomes than count alone.

Poor motility is frequently associated with oxidative stress, mitochondrial dysfunction, and poor testicular circulation.

Acupuncture improves testicular blood flow and reduces scrotal oxidative stress. Chinese herbal formulas for asthenospermia target both the energetic and the biochemical mechanisms driving poor movement.

Abnormal morphology (teratospermia)

Morphology refers to the percentage of sperm with normal shape. A low normal morphology result (below 4% on strict Kruger criteria) means the majority of sperm cannot penetrate an egg efficiently. Morphology is the parameter most influenced by environmental and oxidative factors, which also makes it the most responsive to treatment. Three months of consistent acupuncture and herbal medicine alongside targeted antioxidant supplementation typically produces measurable morphology improvement.

Elevated DNA fragmentation

DNA fragmentation index (DFI) measures the percentage of sperm with damaged genetic material. High DFI  (typically above 15–25% depending on the lab) is associated with failed IVF cycles, poor embryo development, and recurrent miscarriage even when other sperm parameters appear normal.

​

It is frequently overlooked because standard semen analyses do not measure it. If you have had unexplained IVF failures or recurrent pregnancy loss, DFI testing is worth requesting from your RE.

​

Acupuncture reduces DFI through its documented effects on oxidative stress and scrotal temperature regulation. This is one of the stronger evidence areas for male fertility acupuncture.

Hormonal imbalances

​

Low testosterone, elevated FSH, or elevated prolactin can all suppress spermatogenesis. Acupuncture and herbal medicine support hypothalamic-pituitary-gonadal axis function to normalize the hormonal environment for sperm production. This is not a replacement for endocrinology when hormone levels are severely out of range, but for borderline or subclinical hormonal patterns, Chinese medicine produces consistent results.

Libido and erectile function

Reduced libido and erectile dysfunction under the stress of fertility treatment are common and underreported. Both respond to acupuncture.

Stress-related sexual dysfunction is one of the fastest-responding presentations in the clinic.

Most patients notice improvement within four to six sessions.

The 74-day sperm production cycle is the biological basis for the three-month treatment commitment we recommend for all male fertility patients.

​

Weekly or biweekly acupuncture for the first three months is the standard protocol.

 

Sessions address both the systemic pattern (the Chinese medicine diagnosis specific to the individual) and local treatment to improve testicular and pelvic circulation. (No needle goes in the testicles, do not worry)

​

Chinese herbal formulas are prescribed based on your semen analysis results and constitutional pattern. The formula is adjusted as parameters improve.

 

Herbs are not a generic supplement but a precise prescription that changes as the treatment progresses.

​

Supplements are recommended alongside herbs based on identified deficiencies and semen analysis findings. The most commonly indicated are CoQ10 (ubiquinol form), zinc, selenium, vitamin D, and omega-3 fatty acids. Dosages are specific to the individual.

​

Lifestyle factors that directly impact sperm quality are reviewed at intake: scrotal heat exposure (laptops, hot tubs, tight clothing), alcohol consumption, smoking and vaping, anabolic steroid or testosterone use, and sleep quality. Each of these has documented effects on sperm parameters and each is addressable.

Treating Both Partners

Male factor and female factor infertility co-exist in a significant percentage of couples.

 

Treating only one partner while the other goes unaddressed reduces the overall probability of success.

​

At Taproot, we routinely see both partners. This is not a requirement, but when both partners are in treatment simultaneously, the combined improvement in gamete quality is greater than either partner's improvement alone.

 

For couples going through IVF, optimizing both egg and sperm quality before retrieval directly impacts fertilization rates, embryo quality, and live birth outcomes.

For couples using assisted reproduction, male factor treatment has a specific application at each stage.

​

Before IUI: Improving motility and count before an IUI cycle directly increases the concentration of motile sperm available for the procedure. Three months of preparation before a planned IUI is ideal; even four to six weeks produces measurable improvement.

​

Before IVF retrieval: Sperm quality at the time of retrieval determines fertilization rate and early embryo development. Poor morphology and high DFI are leading causes of poor fertilization rates and blastocyst conversion failure. Three months of treatment before a retrieval cycle addresses both.

​

ICSI cycles with poor parameters: Even when ICSI is used (where a single sperm is injected directly), sperm DNA quality still affects embryo development and implantation. High DFI in ICSI cycles is associated with poor embryo progression to blastocyst. Reducing DFI before retrieval improves the biological quality of the embryos created.

FAQ
My (or my partner's) semen analysis was normal. Can acupuncture still help?

A standard semen analysis measures count, motility, and morphology. It does not measure DNA fragmentation, oxidative stress markers, or sperm function.

 

Normal semen analysis with recurrent IVF failure or unexplained infertility is a strong indication to test DFI and consider acupuncture regardless of the standard parameters.

How quickly can sperm parameters improve?

The 74-day sperm production cycle means the earliest you can expect to see parameter changes in a semen analysis is around ten to twelve weeks from starting treatment.

Repeat analysis at three months is the standard benchmark.

 

Some patients see improvement in motility as early as six to eight weeks; morphology and count typically take the full three months.

Do I need to do anything differently during treatment?

Scrotal heat avoidance is the single most impactful lifestyle change: no laptops directly on the lap, no hot tubs or prolonged hot baths, no tight underwear, and no long cycling sessions during the treatment period.

Alcohol reduction and sleep optimization are the next most significant.

Specific guidance is given at your first appointment based on your history.

Is acupuncture uncomfortable for men?

Points used for male fertility treatment are primarily on the lower abdomen, lower back, legs, and arms. Genital points are not used.

Most male patients find the sessions deeply relaxing, often more so than they expected.

Can acupuncture help with azoospermia (zero sperm count)?

Obstructive azoospermia (caused by a physical blockage) requires surgical intervention and is outside the scope of acupuncture.

Non-obstructive azoospermia, where the testes are not producing sperm adequately, has been treated in some cases with Chinese medicine, but results are highly variable and this requires direct consultation to assess appropriateness.

We are already mid-IVF cycle. Is it too late to start?

No. Start now regardless of where you are in the cycle. Even two to four weeks of treatment before a retrieval provides some benefit to sperm quality and reduces oxidative stress at the time of collection. The full benefit requires three months, but partial benefit is better than none.
